Assumption of Risk
I understand that participation in a Wiffle Ball Tournament involves physical activity and may carry a risk of injury or harm. I voluntarily assume all risks, known or unknown, associated with this activity.

Release and Waiver
I, for myself or my child under age of 18, and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the Event organizers, sponsors, volunteers, host location, and their employees or agents (“Released Parties”) from any and all liability, claims, demands, losses, or damages arising out of or related to my participation in this Event.

Medical Treatment
I authorize any licensed physician, emergency medical technician, hospital or other medical or healthcare facility to treat me in the event of any injury or illness resulting from my participation. I assume full responsibility for any and all medical and related costs.

Photographic Release
I grant permission to the Event organizers to use my likeness in a photograph, video, or other digital media in any and all publications, without payment or other consideration.

Participant Certification
I certify that I am physically fit and capable of participating in this activity, and I am not under the influence of any substance that may impair my ability to safely participate.
